To add SELF NFTs to Metamask you need two things, NFT smart contract and the tokenId of your name:

To find your SELF username tokenId you must do the following:
1- Go to dashboard.selfcrypto.io and connect your wallet.
2- Go to "Profile", click "View" in one of your usernames.
3- On the username page if you haven't made any modification since you minted your name, add a new avatar image or some metadata addresses and save the changes on-chain (it will require a TX)
4- You'll find and be able to copy the tokenId of the NFT under the name of the token
